The median nerve is one of the three main nerves that supply the forearm and hand. Because of its position and how often we use our hands for repetitive tasks, median nerve entrapment is common. This nerve travels along the front of the arm, wrist, and hand, providing sensation and motor control to the thumb, index finger, middle finger, and part of the ring finger on the palm side.

What Causes Median Nerve Entrapment?

Entrapment happens when surrounding soft tissues—muscles, tendons, or fascia—compress the nerve. Repetitive or overuse activities such as typing, carpentry, sewing, or playing musical instruments can lead to irritation and pressure on the nerve.

Common symptoms of median nerve compression include:

  • Pain, numbness, or tingling in the thumb and fingers
  • Burning or “pins and needles” sensations
  • Weakness in grip strength
  • Discomfort that worsens with repetitive wrist or hand movement

Carpal Tunnel Syndrome (CTS)

The most recognized form of median nerve entrapment is carpal tunnel syndrome. This occurs when the nerve becomes compressed inside the carpal tunnel, a narrow passage in the wrist bordered by the transverse carpal ligament, carpal bones, and nine flexor tendons. Overuse, poor wrist posture, or repetitive hand movements can reduce space within this tunnel, leading to nerve irritation and inflammation.

Other Causes of Median Nerve Compression

While CTS is common, entrapment can occur higher up the arm. One key site is in the forearm between the two heads of the pronator teres muscle, a condition called pronator teres syndrome. Additionally, neck and shoulder dysfunction may contribute to nerve compression higher along the pathway, producing similar symptoms.
